St. Luke's Medical Center's First International Postgraduate Course in Plastic Surgery

As part of its responsibility in upholding the global standards of surgical excellence as one of the world’s top medical centers and promoting patient safety, the Section of Aesthetic Plastic and Reconstructive Surgery of the Department of Surgery, St. Luke’s Medical Center Quezon City held its First International Postgraduate Course in Plastic Surgery last March 24 and 25, 2011 in the Edsa Shangri-la Hotel, Mandaluyong, Metro Manila. The theme of the conference was “Aesthetic Refinements in Plastic Surgery in the Asian Setting”.

The postgraduate course featured the recent surgical and technological advances in various cosmetic and reconstructive procedures, with an emphasis on how to obtain excellent aesthetic results. It was very informative not only for experienced plastic surgeons, but also to doctors who are still in plastic surgery residency, and plastic surgeons who are new in clinical practice, since there were many lectures that dealt with the most basic aspects of surgical planning and execution, such as pertinent anatomical details, pre-operative
considerations, and basic plastic surgical techniques. But, pursuant to its goals of promoting medical science and education and improving patient safety, the postgraduate course was also open to all doctors who are interested in knowing more about cosmetic surgery.

Distinguished foreign plastic surgeons and the Active Plastic Surgery Consultant Staff of the St. Luke’s Medical Center (Quezon City and Global City) spoke on various topics of interest, particularly in the different subspecialties of plastic surgery, such as craniofacial surgery, facial aesthetics and facial rejuvenation, blepharoplasty, rhinoplasty, breast augmentation, mastopexy, breast reconstruction, management of chest wall deformities, liposuction, abdominoplasty techniques, body dysmorphophobia and minimally-invasive
surgical techniques.

There were six invited speakers from other institutions, namely: Dr. Darryl Hodgkinson from Sydney, Australia; Dr. Susumu Takayanagi, from Osaka, Japan; Dr. Ashok Gupta, from Moombai, India; Dr. Chang-Chien Yang, from Chinese Taipei; Dr. Apirag Chuangsuwanich, from Thailand; and Dr. Florencio Lucero, Secretary-General of the Oriental Society of Aesthetic Plastic Surgery. The rest of the faculty speakers were all aesthetic plastic and reconstructive surgeons who are currently practicing at St. Luke’s Medical Center (Quezon City/Global City): Dr. Dale Abraham, Dr. Briccio Alcantara, Glenn Genuino, Dr. Marlon Lajo, Dr. Laurence Loh, Dr. Melvin Sibulo, Dr. Bernard Tansipek and myself.

A total of 25 lectures on different areas of plastic surgery were delivered. On March 24, the lectures were organized into four main sessions: the Facelift Session, Craniofacial Session, Blepharoplasty Session 1 and Blepharoplasty Session 2. On March 25, lectures were distributed into five sessions: Rhinoplasty Session, Breast Session 1, Breast Session 2, Abdominoplasty Session, and finally, the Liposuction & Body Implants Session.

The following is the list of lectures that the participating plastic surgeons delivered:

Top Cosmetic Surgery Procedures Worldwide

Global statistics on cosmetic surgery procedures performed in 2009 released last year by the International Society of Aesthetic Plastic Surgery (ISAPS) showed that liposuction and breast augmentation were the top cosmetic surgery procedures performed worldwide.

The total number of cosmetic surgical procedures was 8,536,379 with liposuction as the top procedure worldwide, representing 18.8%, followed by breast augmentation (17%), blepharoplasty (upper or lower eyelid lift) at 13.5%, rhinoplasty (9.4%) and abdominoplasty (tummy tuck) at 7.3%.

Of the top 25 countries with the most number of cosmetic surgeries done, liposuction was the top procedure in the U.S.A. (221,160), Brazil (253,302), India (117,140), Germany (57,761), Italy (43,250), Russia (37,137), Spain (33,073), France (28,272), Colombia (16,420), United Kingdom (14,697), Venezuela (15,429), Portugal (12,186) and Belgium (12,003).

On the other hand, breast augmentation was the top cosmetic surgery done in China (222,530), Mexico (101,402), Japan (75,294), South Korea (66,864), Turkey (41,797), Argentina (27,918), Canada (28,178), Taiwan (18,902), Thailand (13,875) and Saudi Arabia (11,833).

Statistics compiled by the American Society of Plastic Surgeons (ASPS) however showed a different trend. ASPS statistics showed breast augmentations to be the most frequently performed cosmetic surgery
procedure in the USA. ASPS data show that 289,328 breast augmentations were performed in the U.S.A. in 2009, and this figure increased to 296,203 in 2010.

The discrepancy can be attributed to the fact that the ISAPS global survey was based on statistics compiled from board-certified plastic surgeons only, while the survey conducted by the ASPS included data from board-certified plastic surgeons, dermatologists and otorhinolaryngologists.

There was no doubt however that the U.S.A. was the top country worldwide where the most cosmetic surgeries were done (1,303,932), followed in decreasing order by China (1,215,245), Brazil (1,054,430), India (683,160) and Japan (411,182). Next down the list were Mexico, Japan, South Korea, Germany, Italy, Russia, Turkey, Spain, Argentina, France, Hungary, Canada, Colombia, United Kingdom, Taiwan, Venezuela, Thailand, Australia, Portugal, Belgium, and Saudi Arabia.

The ISAPS Global Survey also revealed a most interesting fact: nonsurgical procedures done (8,759,187) actually outnumbered surgical procedures (8,536,379). The top nonsurgical procedure done was botulinum toxin injections (32.7%), followed by hyaluronic acid filler injections (20.1%), laser hair removal (13.1%), autologous fat injections (5.9%) and IPL laser treatment (4.4%). Next on the list were the following procedures: chemical peel, microdermabrasion, laser skin resurfacing, sclerotherapy and laser treatment
of leg veins.

Nonsurgical procedures were most popular in the United States, Brazil, Mexico, China, Japan, Hungary, South Korea, India and Germany. The results of the ISAPS Global Survey confirm how popular the non-invasive procedures have become.

In the Philippines, there is unfortunately no credible and reliable baseline of information regarding both surgical and nonsurgical cosmetic procedures. But informal surveys show the popularity of rhinoplasty, breast augmentation, blepharoplasty, liposuction and abdominoplasty among the surgical procedures. For nonsurgical procedures, botulinum toxin injections, hyaluronic acid injections, chemical peels and laser skin resurfacing are among the most commonly performed.

My new year has gotten off to a great start! I have just learned that my application to be an International Active Member of the American Society of Aesthetic Plastic Surgery (ASAPS) has been accepted. The ASAPS is the most prestigious organization of board-certified plastic surgeons who specialize in cosmetic plastic surgery and to be one of only two members in the Philippines is a distinct honor.

The mission of the American Society for Aesthetic Plastic Surgery is "to advance the science, art, and safe practice of aesthetic plastic surgery among qualified plastic surgeons." This includes medical education, public education and patient advocacy. Indeed, whenever I attend the annual ASAPS conferences, the latest advances in cosmetic plastic surgery are presented and patient safety is always discussed.

I am proud to be a member of an organization that promotes and encourages "the highest standards of ethical conduct and responsible patient care among aesthetic plastic surgeons", especially because the requirements for membership are quite stringent. To quote from the ASAPS website:

"The requirements for membership in the American Society for Aesthetic Plastic Surgery (ASAPS) are designed to encourage application by trained and experienced plastic surgeons who concentrate their practices in performing cosmetic plastic surgery of the face and the entire body. ASAPS membership remains an exclusive privilege for those surgeons who possess the necessary qualifications. Only about one-quarter of all American Board of Plastic Surgery certified surgeons have been accepted into ASAPS membership. Physicians who have been trained in specialties other than plastic surgery are not eligible for membership in ASAPS."

Last year, I also became an International Member of the American Society of Plastic Surgeons, a Member of the International Society of Aesthetic Plastic Surgery and a member of the Oriental Society of Aesthetic Plastic Surgery. I am looking forward to the year ahead and the opportunities for learning that all these excellent organizations provide. Happy new year!

ISAPS 2010: The Global Summit on Patient Safety

Last August 14, 2010, the International Society of Aesthetic Plastic Surgery inaugurated the ISAPS Global Summit on Patient Safety. With speakers from various specialties, including legislators, travel and insurance representatives, as well as plastic surgeons, this was definitely a very interesting forum for me.

The growing commercialization of aesthetic surgery has resulted in many more cases being done each year compared to the past. In the past few years, we have heard many reports of cases of cosmetic surgery tragedies. The case of the Sylmar sisters who did silicone injections in the buttocks which resulted in a woman's death, and the Donda West case, wherein the mother of singer Kanye West died from complications of cosmetic surgery, are just some of these. Although many of these procedures were performed by people who are not qualified, board certified plastic surgeons, many of the people who read the news reports do not know the difference between a board certified plastic surgeon and someone who just decides to do cosmetic procedures even without the proper training. It is a challenge for us to educate patients about this. Although some may say the concern of plastic surgeons is mainly about the issue of other doctors practicing our specialty, for us, it is all about patient safety.

Aesthetic surgery is a surgery of want rather than need. It is not like other surgeries where people are sick and they have to undergo certain procedures. They just have to accept the risks that go with the procedure they need. Perfectly normal people undergo many aesthetic procedures every year. How can we protect them and reduce these tragic cases?

Part of the forum focused on the legislation that has been done in countries like France and Denmark. In France, they have succeeded in initiating changes and today, there are strict regulations about who can perform procedures. Specialists are only permitted to perform plastic surgery in the part of the body they have specialized in. They are allowed to perform procedures only in their area of specialty. They also have other regulations regarding the presence of paramedical personnel, and the mandatory waiting period after being given a quotation by the surgeon. Other countries like Denmark, Norway and Singapore also have guidelines and policies protecting the patient. Germany also has a ban on advertising for aesthetic surgery. There is no such legislation in the Philippines. And although I have said it before, I will say it again - in the Philippines, it is legal for any doctor, whatever training they have undergone, to do cosmetic surgery. If patients knew what is involved in aesthetic surgeries, I doubt whether anyone would disagree with me that for greater patient safety, it is best to have strict guidelines about who may perform these surgeries.

I really hope that in the future, the Philippines will catch up with other countries in protecting aesthetic surgery patients through legislation.

ISAPS Congress 2010

The International Society of Aesthetic Plastic Surgery is celebrating its 40th anniversary during the 20th Biennial Congress on August 14, 2010 to August 18, 2010 in the Moscone Convention Center and San Francisco Marriott Hotel, San Francisco, California, USA.

The congress provides the latest in aesthetic surgery education and board certified plastic surgeons from more than 80 countries are attending.

The ISAPS Global Patient Safety Summit took place on the first day of the congress and topics such as Global Regulations, Medical Procedures Abroad and Achieving Global Patient Safety were discussed. Since most of my practice is composed of patients from abroad, these topics were of special interest to me.

Various specific procedures such as Periocular Rejuvenation, Facial Rejuvenation, Rhinoplasty, Aesthetic Breast Surgery, Aesthetic Reconstructive Surgery, and Abdominoplasty are covered during the five day congress. I have found that there is always something new to learn even in the procedures I have been doing for many years when colleagues share their particular way of handling a procedure. As I always say, continuing education is an important part of a plastic surgeon's responsibilities as we strive for excellence, progress and patient safety.

Cosmetic Surgery for Teenagers

The recent news about Charice Pempengco receiving Botox and Thermage treatments has stirred up controversy worldwide. The controversy centers mainly on the question on whether teenagers should be undergoing cosmetic treatments at a relatively early age. In my practice, I do not encourage cosmetic surgery in teens unless there are serious issues about a teenager's self-esteem that can only be corrected with a surgical procedure. If there is a history of the teenager suffering emotionally because of a body feature such as a saddle nose or a flat chest, there may be justification for performing cosmetic surgery to improve the teenager's self-esteem.

In the case of the procedures Charice Pempengco underwent, we are not dealing with deformities brought about by a congenital deformity (such as cleft lip or palate) or due to trauma (such as car accidents or burns) but instead, doing procedures on a normal-looking teen. There have been conflicting reports about why she underwent these procedures. If she was experiencing jaw pain, this can be an indication for treatment with botulinum toxin injections. If done for cosmetic reasons, the question is whether she underwent these procedures because she herself was bothered about the fullness of her face. I believe that a doctor should not convince a patient that she needs a certain procedure if she herself does not bring up a concern about a certain feature. Even parents should not convince their children to undergo cosmetic procedures. It can be harmful to a child's self esteem to be told she/ he needs a cosmetic procedure.

Aside from the issue of Charice Pempengco's age, another concern is the fact that the procedures were done publicly, with entertainment reporters witnessing the event and writing about it. As a plastic surgeon, this concerns me because we don't want to send the wrong message about these procedures. First of all, we don't want full-faced individuals, especially teenagers, to come to the conclusion that having a full face is something that needs correcting. Because she is a public figure and she underwent these procedures publicly, Charice Pempengco seems to have given this message, even if unintentionally.

Also, doing the procedures publicly seems to trivialize cosmetic procedures because people can get the impression that these are beauty parlor procedures. In my practice, I have seen many patients who have undergone some procedures in places like beauty parlors and who require some kind of corrective surgery. Cosmetic procedures should not be taken lightly and even seemingly simple procedures like nose augmentation and eyelid surgery can have serious complications when not done by qualified, board certified doctors.

The four day annual Aesthetic Meeting of the American Society of Aesthetic Plastic Surgeons and the Aesthetic Surgery Education and Research Foundation for 2010 truly lived up to its theme of being “A Capital Experience with a Global Perspective.”

The scientific program included scientific sessions and teaching courses on topics such as the Role of Biofilms in Implant related Complications, Hot Topics in Aesthetic Surgery, Mastopexy Augmentation: Making the Right Choices, Optimizing Outcomes in Breast Augmentation, New Devices in Plastic Surgery, The Pinch Blepharoplasty for Safer and Superior Results and many more. After each session, participants were given the opportunity to ask questions, to provide for an interactive learning experience.

Among the international faculty of key opinion leaders were Renato Saltz, Jack Fisher, William Adams, Bahman Guyuron, Foad Nahai, and many more world renowned plastic surgeons. Exhibits featuring the latest developments in the field of aesthetic surgery were held at the Prince George’s Exhibition Hall of the Convention Center. The opportunity to see the latest in products and services was also an educational experience for the participants.

Before the conference proper, there were optional courses and the Allergan academy, which gives continuing education about Breast Surgery. This year’s theme was “Optimizing Outcomes in Revsion Breast Surgery”.

The annual Aesthetic Meeting of the American Society of Aesthetic Plastic Surgeons is considered by many aesthetic plastic surgeons to be the most important convention of the year.

Breast Augmentation Options

Anyone who does some research on Breast Augmentation will find out that there is a wide range of options available to a patient considering this procedure. Breast augmentation is definitely not a "one size fits all" procedure and for good results, a plastic surgeon must help his patient understand these options.

The choice of breast implants is based on a variety of factors. Her physical characteristics (body measurements, skin), patient preference for breast profile and budget are among the important factors to consider. There are so many implants available, made by various manufacturers, in different price ranges. The plastic surgeon must choose the proper implant to give the best results for his patient. Other decisions to make include where to make the incision (which determines where the scar will be), and what level to position the implant. The problem with doctors who practice plastic surgery without the proper training is that with their limited knowledge and experience, they may not be able to make the best decision about the different options available.

Plastic surgeons are not miracle workers and we cannot promise perfect results. Contrary to how media sometimes portrays plastic surgery, even the best surgeons can have some complications. Thus, the best plastic surgeons will not promise their patients a risk free operation. However, with their specialized training and years of experience, complications are usually minimized and dealt with accordingly if ever they occur. Less experienced doctors who lack the proper training may not be able to deal with complications properly. Also, every year, in the various international plastic surgery conferences, there are updates regarding minimizing complications. Many of these conferences are open exclusively to plastic surgeons.

Asian Breast Aesthetic Symposium Held in Malaysia

I recently attended the first Asian Breast Aesthetic Symposium, with the theme “Taking it to the Next Level” held at the Gleneagles Intan Medical Centre in Kuala Lumpur, Malaysia last November 5 to 7, 2009. I had the honor to be one of three Filipino plastic surgeons to participate in this ‘by-invitation only’ event which focused primarily on aesthetic surgery of the breast. The symposium included various lectures as well as live surgery on closed circuit video. Participants from Malaysia, Thailand, Vietnam, Japan, Hongkong, China, India, New Zealand, Korea and the Philippines attended the event.

During the live surgery sessions, Dr. Charles Randquist (Sweden), Dr. Marco Faria Correa (Brazil and Singapore), Dr. Luan Jie (China), and Dr. Yoshinori Nagumo (Japan) demontrated a variety of surgical techniques in breast augmentation. Dr. Charles Randquist performed breast augmentation using the latest form stable cohesive silicone gel implants, using inframammary fold incisions. Dr. Marco Faria Correa used polyurethane breast implants with periareolar incisions. Dr. Luan Jie performed transaxillary dual plane breast augmentation with the use of an endoscope while Dr. Yoshinori Nagumo also used transaxillary incisions, but without the endoscope. Dr. Steven Teitelbaum and Dr. Arthur Swift were moderators as we participants were given the opportunity to interact with the surgeons while they performed these live operations.

Watching other surgeons perform their surgeries showed that there is not just one perfect way to do surgical procedures, and there are advantages and disadvantages for each of the various techniques. There were variations in the way the surgeons approach implant selection, patient markings, location of incisions, plane of dissection, dissection, equipment/instruments, antibiotics irrigation, use of drains, ways to help prevent complications. Each surgeon had his own technique which from experience gives him the least complications.

Aesthetic breast surgery is one of the most popular procedures in aesthetic surgery today and the symposium aimed to give participants a venue to learn the latest advances in this particular procedure. Among the topics covered were the aesthetics of the breast, factors in implant selection, advantages and disadvantages of the various techniques, cultural differences, minimizing scarring, ways to lower complication and reoperation rates, evidence based medicine and best practices for safe and predictable short and long term results with the least complications. The event allowed for discussion which concentrated on the many details of aesthetic breast surgery which otherwise cannot be covered by a less focused event which covers many other procedures.

13th Turkish Society of Aesthetic Plastic Surgery Congress and ISAPS Course

I recently attended the 13^th Annual Congress of the Turkish Society of Aesthetic Plastic Surgery. Held in conjunction with the International Society of Aesthetic Plastic Surgery (ISAPS) Post Graduate Course, it took place in Istanbul, Turkey last June 19 -22, 2009. International delegates from Macedonia, Greece, Iran, Iraq, Lebanon, Greta Britain, Germany, Spain, Holland, Belgium, USA, Venezuela, Brazil, China, Singapore, New Zealand, Australia, South Africa and the Philippines participated in the four day event. New Laser Technology Demonstrated in Liposuction Workshop

The Philippine Association of Plastic, Reconstructive and Aesthetic Surgeons (PAPRAS) conducted a workshop on liposuction last November 18, 2006. A series of lectures were given on various topics: history and development of liposuction, current liposuction techniques, new technologies, particularly the new Erchonia laser, and autologous fat injection into the facial muscles for facial rejuvenation. The program also included an actual live liposuction demonstration on a female patient regarding, with particular focus on surgical planning, preoperative use of the new Erchonia laser on the target areas for liposuction, and surgical techniques.

Of particular interest to the participants was the aforementioned Erchonia laser. It is a laser with low-level (or cold) output with no thermal effect which will destroy tissues. It is purported to actually emulsify fat tissues. When used just prior to liposuction, it supposedly makes the removal of fatty tissues during liposuction significantly easier and quicker. Clinical trials showed that patients have less pain immediately following surgery. More fat can be removed per minute using the Erchonia laser, which means shorter time for doing the liposuction. No complications such as burning of tissues, seromas or other side effects were reported in the clinical trial.

PAPRAS is the only association of plastic surgeons that is formally recognized by the Philippine College of Surgeons. An interesting topic that Dr. Foad Nahai presented at the 13th ASEAN Congress of Plastic Surgery was about the Prevention and Management of Complications in Aesthetic Surgery. With a touch of humor, he stated, "If a plastic surgeon claims he hasn't had any complications, he is either not telling the whole truth or he hasn't had that many operations." A humorous statement but I would say I would agree with him. Dr. Nahai, who is highly esteemed among plastic surgeons as the author of many books on plastic surgery, humbly and candidly shared some of the complications he encountered in his own private practice and generously shared valuable information about how to lower the risks of complications and how to manage complications. I'm sure many of the younger and less experienced plastic surgeons found his talk especially interesting.

Plastic surgeons who have practiced for a long time and who have had years of experience know that although serious complications are rare, there are complications that they have to be ready to manage. This is precisely why it is of primary importance that patients choose their plastic surgeons carefully. Although many doctors choose to do cosmetic surgery without the specialized training that plastic surgeons have, they may only be familiar with the uncomplicated surgery case and may lack the experience to handle complications well.

The Cutting Edge in Aesthetic Plastic Surgery

The keynote lecture in the 13th ASEAN Congress of Plastic Surgery was given by world renowned plastic surgeon, Dr. Foad Nahai, who I had the opportunity to observe when I was training in 1990 with Dr. John Lewis, founder and first president of the American Society of Aesthetic Plastic Surgery. Dr. Nahai was with the Emory University Hospital in Atlanta, Georgia then.

Dr. Nahai's lecture was interesting because, contrary to popular opinion that the cutting edge in aesthetic surgery means that it is the newest and the best, he stated that the "cutting edge" could be a fad or an unproven technique, therapy or procedure and therefore, not necessarily the best. Some of the newer techniques that he mentioned were now under review were radiofrequency skin tightening, barbed sutures, and nonsurgical liposuction alternatives. He gave a quote that goes something like this: "Not everything that is new is good, but everything that is good was once new." And so, this highly esteemed plastic surgeon cautioned the participants to the Congress and said, "Don't be the first to jump on the bandwagon of something new. Not the last either." With a touch of humor, he warned us "Don't fall off the cutting edge".

Dr. Nahai noted that the world wide interest in cosmetic surgery has never been greater. This is due to the exposure from media. According to him, media coverage may be considered good because it increases awareness and intrest but in a way, it can also be bad because it can lead to unrealistic expectations. Sometimes, media tends to minimize the risks of plastic surgery.

Some statistics that he cited: In the U.S., $12.4 Billion was spent on cosmetic surgery, with a growing trend towards non-surgical options with less downtime. With all the opportunities and challenges the plastic surgeon faces today, Dr. Nahai encouraged the plastic surgeons in attendance to "Continue to hold ourselves to the highest standards, always put the patient's interests first, enforce standards and weed out "bad eggs", educate the media and the public and to continue to be a real plastic surgeon and not just a cosmetic surgeon."

On the topic of Body Sculpturing, Dr. Yeap Choong Lieng presented Liposelection, The Revolutionary Technique, Dr. Gwendy Aniko spoke about Water-Jet Assisted Liposuction, and Dr. Benjamin Ascher discussed Lipolysis by External Ultrasounds. Speakers for the topic of Rhinoplasty were Dr. Florencio Lucero from the Philippines, and Dr. Montri Kitmanee and Dr. Yu-Ray Chen.

These are just some of the many topics which were presented. I and the other Congress participants greatly benefitted from the sessions during this congress as the presentors generously shared their experiences and techniques with their collegues.

Plastic Surgery Around the World

The invited speakers to the 13th ASEAN Congress of Plastic Surgery were plastic surgeons from Thailand, United States of America, Taiwan R.O.C., Singapore, Indonesia, Australia, Austria, Japan, Korea, the Philippines, Switzerland, Israel and Italy. As the various topics were discussed by these surgeons, it was interesting to note the differences and commonalities in the practice of plastic surgery in the different countries. For example, since 1992, plastic surgeons from the United States have only been able to use silicone gel implants under certain conditions such as for the purposes of reconstruction after mastectomy. In contrast, silicone gel implants, which many of the American surgeons prefer over the saline implants they are allowed to use, are widely used in Asian countries such as the Philippines. Thus, patients who prefer the silicone gel implants often have to travel to countries where these are used to have their surgery.

Another example of differences would be the type of rhinoplasty (nose) surgery which is commonly done in the different countries. While nose surgery in the United States is predominantly for reduction, in the Philippines as well as other Asian countries, augmentation rhinoplasty or the nose lift would be the more common procedure. Thus, when deciding to have these procedures done, it is important to consider the experience the plastic surgeon has. Asian doctors usually have more experience in augmentation, and if considering having reduction rhinoplasty done in an Asian country, a plastic surgeon who has had training in countries like the United States is the preferred option.

The 13th ASEAN Congress of Plastic Surgery was held at the Sheraton Chaing Mai Hotel, Chiang Mai, Thailand last October 16 - 18, 2006. The theme of the conference was "The Cutting Edge in Plastic Surgery and Charm of the North". In attendance were plastic and reconstructive surgeons from around the world, most of us from the Asian nations. Among the topics discussed were Breast Reduction, Breast Reconstruction, the Personal Approach to Total Facial Rejuvenation, Body Sculpturing, Rhinoplasty & Blepharoplasty. There are many new developments in the field of plastic surgery and there are always many things plastic surgeons can learn from one another during these international conferences. Available Breast Implants in the Philippines

How would you feel if you traveled to a foreign country to have breast augmentation and the specific breast implant you prefer is not available there? The purpose of this article is to inform prospective patients about the kinds of breast implants that are available in Metro Manila, Philippines.

There are various kinds of breast implants. In general, the most commonly used are filled with either silicone or a saline (salt water) solution. The soft outer shell is either smooth or textured. There are various sizes available, as well as shapes and profiles.

In the Philippines, the patient has a choice of the following brands: McGhan, CUI, Mentor or Eurosilicone. In general, round smooth surfaced or textured surfaced implants are readily available and the patient does not have to make a special orders.

If the patient desires anatomically shaped implants, please be advised that these are not in stock by distributors of all the major brands of implants. A special order has to be placed, and it takes from 2 to 3 weeks before the anatomical implants can be delivered.

The Dangers of Silicone oil injections for Non-surgical Breast Augmentation and Noselifts

Have you ever held a bag of marbles in your hand? Believe it or not, this was how my patient's breasts felt like. She had multiple lumps all over her breasts, the result of multiple injections of silicone oil given one year ago. She felt insecure about her small breasts and she wanted them to be enlarged. Someone told her of a method that was fast and cheap. Nobody told her she was going to end up this way.

I have seen a number of patients who have been victimized by glib-talking people extolling the virtues of silicone oil injections. The primary attraction is the cost: "only" P40,000 for breast enlargement, which is less than half of the average cost for breast augmentation with medical-grade silicone gel implants. For those desiring a noselift, the cost is from P7,000 to P14,000. Victims are often "fast-talked" into undergoing the procedure; they are not given the time to consult with their doctor.

Noselifts and breast augmentations are two of the most commonly requested cosmetic surgeries, but there are many who find the expense quite prohibitive. Thus the lure of low-cost injections is hard to resist. When the complications arise, these victims end up spending more.

Those whose noses are injected with silicone oil end up with noses looking like the noses of lions - as the silicone oil spreads through the tissues with time, the bridge of the nose flattens out and becomes wider. The skin is often reddish, a result of chronic inflammation of the tissues.

The people who perform silicone oil injections are hard to catch. They do not have any permanent offices. They go from one beauty parlor to another; they transfer from house to house, going wherever there is someone gullible enough to fall for their sales pitch.

I hope that with articles like this, people will become aware of the dangers of silicone oil injections so that they can avoid its consequences.

Medical Tourism in the Philippines

Philippine officials formally launched a Medical Tourism campaign last January to promote the country as "islands of wellness." Health Secretary Francisco Duque III said the program, participated in by government agencies, private clinics and hospitals, aimed to make the Philippines the "new hub of wellness and medical care in Asia".

The components of the program include medical, surgical and dental care, traditional and alternative health care, health and wellness, long-term tourism and the establishment of international retirement and medical zones.

Medical tourism, which combines both health care and travel and leisure, is a multi-billion dollar industry and the Philippines is aiming to compete directly with other Asian countries such as Thailand, India and Malaysia. The program aims to target medical tourists from the United States, Europe, Australia, South Korea, Japan and Pacific island states, especially the large Filipino expatriate and immigrant community.

"We believe that the Philippines can tap into the remaining huge market potential." Duque said. "I think we still have very many worthy qualities as health care providers, and our medical and surgical capabilities are quite comparable, if not superior," he said.

Duque also mentioned that price advantage available to medical tourists for medical and surgical procedures in the Philippines, which are 30 to 50 percent cheaper than elsewhere. A coronary bypass costing about $50 000 in the United States is only about $25 000 in the country, with comparable clinical expertise and facilities, as well as complication and success rates, he said.

Continuing Education for Plastic Surgeons: An Essential

When I went to Chicago last year to attend the annual conference of the American Society of Plastic Surgeons, I had the privilege to attend the INAMED Academy, an educational course provided to board-certified and board-eligible plastic surgeons, and plastic surgery residents by INAMED Aesthetics, (formerly McGhan® Medical Corporation). The stated mission of INAMED Academy is “to educate through a unique forum that reinforces product knowledge, encourages practice innovation and allows for the exchange of new ideas”.

INAMED has been in the business of manufacturing and marketing of breast implants, tissue expanders, and dermal filler products for the past 25 years. The company has pioneered innovations such as the patented INTRASHIEL® Barrier Shell, the BIOCELL® Textured Shell and unique silicone-fill technology. The afternoon lectures centered on breast implants. Among the topics discussed were the following: “FDA Update”, “Rationale for Form Stable versus Non-Form Stable Breast Implants”, and “Personal Experience Using the Style 410”. This was then followed by an Interactive Panel which discussed breast surgery complications, management of breast asymmetry, and expander-assisted breast reconstruction.

The course faculty was composed of distinguished plastic surgeons, namely, Scott Spear, G. Patrick Maxwell, Mark Jewell, Steven Teitelbaum, Per Heden, and several others.

Continuing education is very important for medical professionals and it is essential for plastic surgeons to avail of opportunites like the INAMED Academy to keep up to date with the many developments in plastic and cosmetic surgery products and techniques. Plastic Surgeon or Cosmetic Surgeon: What's the Difference?

With the increasing popularity of cosmetic or aesthetic surgery, patients often encounter different terms such as "plastic surgeon", "aesthetic plastic surgeon", "cosmetic plastic surgeon", "plastic and reconstructive surgeon", "cosmetic surgeon" or "aesthetic surgeon". Is there any difference whether a patient chooses a plastic surgeon or a cosmetic surgeon to perform the procedure they are interested in? The answer is YES, there is a difference.

Patients considering plastic surgery should know that in the Philippines, it is legal for physicians who are NOT PLASTIC SURGEONS to perform cosmetic surgery. Once doctors get their medical degrees, they may practice any specialty, even if they have not completed advanced training in the field they have chosen. And so, a doctor who performs cosmetic plastic surgery is not necessarily trained as a plastic surgeon.

Many documentaries have been made and many horror stories have been told about plastic surgery that went terribly wrong , but in most cases these procedures were done by unskilled and unqualified physicians.

Plastic surgery - like any surgery - carries risks and must be treated seriously. To ensure the best results for the surgery you are considering, choose an authentic plastic surgeon with the training and experience that is essential for the success of your surgery. Choosing the right surgeon is the single most important decision a patient considering plastic surgery will make.

Plastic Surgeons

The primary difference between a plastic surgeon and a cosmetic surgeon is the training that they have undergone.

To become a plastic surgeon and use the title of "Plastic Surgeon" in his clinical practice, a doctor must be fully licensed to practice medicine and must undergo at least five to six years of a formal residency training program in Plastic & Reconstructive Surgery that is accredited by the Philippine Board of Plastic Surgery, in a hospital or medical center that is accredited by the Philippine College of Surgeons to provide such training. This includes three to five years of general surgery residency and two to three years of plastic surgery residency. Doctors who have completed residency training in plastic and reconstructive surgery are the only doctors who can rightfully claim to be plastic surgeons.

A fully trained plastic surgeon can therefore use any or all of the following titles: Plastic Surgeon, Plastic & Reconstructive Surgeon, Aesthetic Plastic Surgeon, Cosmetic Plastic Surgeon, Facial Plastic Surgeon, Aesthetic Surgeon, and Cosmetic Surgeon.

Cosmetic Surgeons

On the other hand, cosmetic or aesthetic surgeons are doctors who perform cosmetic surgery procedures, regardless of the training they have received. Thus, the use of the titles "Cosmetic Surgeon" or "Aesthetic Surgeon" may mean many different things. The patient must inquire into the training of the doctor because these doctors have varying credentials, such as residency training in general surgery, ENT, OB-Gyne, dermatology, or the doctor may not even have formal surgical training.

In the Philippine setting, it is usually a doctor who practices cosmetic surgery and who has not undergone an accredited formal residency training in plastic and reconstructive surgery, who uses the title of "Cosmetic Surgeon" or "Aesthetic Surgeon". From an ethical point of view, such doctor should not use a title containing the word "Plastic" because only those with accredited training recognized by the Philippine College of Surgeons are entitled to do so.

Doctors who call themselves "Cosmetic Plastic Surgeons" or "Aesthetic Plastic Surgeons", but have not undergone residency training in plastic and reconstructive surgery, are misleading the general public and engaging in unethical medical practice.

Choosing a Cosmetic or Plastic Surgeon

The extensive training that a plastic surgeon undergoes gives him a definite advantage over a cosmetic surgeon who has not had this specialized training.

Mere familiarity with the anatomy of a certain body region does not automatically give a doctor the skill to perform cosmetic surgery in that area. The principles of plastic surgery require years of training and experience to be mastered.

General surgeons have more training in doing intestinal resections, appendectomies, cholecystectomies, mastectomies,thyroidectomies and hemorrhoidectomies than in doing cosmetic surgery. Dermatologists have more training in the medical treatment of skin disorders and often do not have training in the surgical treatment of skin lesions. Ear, nose and throat specialists and obstetrician-gynecologists have more training in their fileds of specialty than in doing breast augmentations, facial surgery and liposuction.

A doctor who practices cosmetic surgery must not only know how to do a procedure, but he must also know what to do in case wound healing does not proceed normally or when other complications arise. The specialty of plastic and reconstructive surgery has a major focus on the treatment of difficult wounds and chronic non-healing ulcers. Surgery is not an exact science and the body may react in unpredictable ways. A plastic surgeon is trained to respond to unexpected outcomes.

In addition to possessing an array of technical skills to use when dealing with complications, a plastic surgeon develops a sense of form, an aesthetic judgment, and an ability to visualize the end result. His training and years of experience make a definite difference in the quality of work and success of the surgery.

Dimples, Anyone?

Cheek dimples have long been considered an attractive feature in the face.

The surgical procedure used to create dimples is, in itself, a fairly simple one and is commonly done on an outpatient basis. The procedure involves simulating the natural way a dimple is formed, by forming a connection between the skin and the cheek muscle, such that when the muscle contracts, it pulls the skin inwards to form a dimple.

So how exactly is a dimple created?

A punch biopsy instrument is placed against the buccal mucosa (inner cheek) and circular motions are made to cut through the mucosa, submucosal fat, and cheek muscle. A circular core composed of these tissues is removed, leaving the skin intact, which will create a shallow cylindrical-shaped defect under the skin.

This 'defect' then is closed by placing an absorbable suture through the cheek muscle on one side of the defect, then through the dermis layer of the skin and finally through the cheek muscle on the othe side of the defect. A surgical knot is tied, which in turn will result in dimpling of the skin even without smiling.

The skin will, however, flatten out in a week or two when the suture is absorbed. The internal scarring which takes place will connect the muscle to the skin, creating a dimple when one smiles.

The surgery can be performed on any kind of cheek, be it chubby or not. It would be easier if the cheeks are not too chubby. If someone wants dimples that are very prominent, a bigger punch biopsy will be used to be able to remove a bigger core of soft tissue.

Usually performed under local anaesthetic, the surgery takes about 30 minutes, and normal activities can be resumed straight after the procedure. You walk in without a dimple and leave with your desired facial indents.

There is usually minimal swelling after, and the after care steps are just as simple and uncomplicated as the procedure. The after care usually involves regular mouth rinsing with Betadine oral antiseptic solution or Bactidol (chlorhexidine) solution, and antibiotic intake.

How to Look Good this Summer Without Really Trying

Summer vacations in an archipelago like the Philippines are often spent on the beach, frolicking in the water, enjoying the sun and surf, or idling the time away on the sand just watching other people have fun. To many people, the coming of summer means a return to the gym or health club in order to whip one's body back to shape, shedding excess pounds, burning the fat away, in order to look as good as possible in skimpy bikinis or swimming trunks.

For many who do not have the time nor the motivation to spend hours working out in the gym, cosmetic surgery offers a quick solution to the problem of how to look good in swimwear without having to exercise. The popularity of cosmetic surgery coupled with its increasing affordability has made cosmetic surgery a viable option for many who desire an attractive body shape.

What better way is there to get curvy or flat contours other than liposuction? In 2005, it is estimated that more than 270,000 people underwent liposuction in the U.S.A. alone, making it the most popular cosmetic surgery procedure. Such big numbers indicate a desire of a large number of people to have an attractive figure. For men, this generally means having a flat abdomen coupled with straight flanks on both sides. Women on the other hand undergo liposuction to get thinner waistlines and to remove localized fatty bulges that are most difficult to get rid of even with rigorous exercise. Areas where fat deposits are the hips, thighs, abdomen, and buttocks, as well as on arms, knees, calves, upper torso, neck and under the chin.

Liposuction is a safe procedure when done by a well-qualified plastic surgeon. Patients with young, healthy elastic skin who have localized areas of fat are the best candidates for liposuction. This procedure can be done on an outpatient basis, if the fat to be removed totals 5 liters or less. (For amounts greater than 5 liters, it is recommended that patients stay a night or two in the hospital). After liposuction is performed, patients are advised to wear compression garments for 2 weeks, in order to prevent accumulation of blood (hematoma) or serum (seroma), and to enable the skin to adhere to underlying tissues.

There may be some swelling and bruising after liposuction, but these usually subside within 6 to 8 weeks. Patients are advised to restrict their activities for the first two weeks following surgery. Sports activities are resumed gradually on the third postoperative week. Direct sun exposure of liposuctioned areas should be avoided until all bruising has disappeared, usually within 2 to 4 weeks. This schedule for gradual resumption of normal routine should be kept in mind by those who would like to be active on the beach by the time summer starts.

The second most popular cosmetic surgery procedure is breast augmentation. Current norms of beauty regard women with full shapely breasts and thin waists as more attractive than those with breasts of lesser dimensions. It is no secret that women with bigger breasts will get more admiring looks from men compared to those with flatter chests. Do you still wonder why "Baywatch" is the most watched television show all over the world?

Following breast surgery, patients may have minimal to moderate pain which is controlled by painkiller medications. Antibiotics are also prescribed to prevent infection. Patients are advised bed rest and limited activities for the first few days after surgery. Sutures are removed in 5 to 7 days. After one week, massage of recommended to keep the breasts supple. There may be numbness around the treated area but this is usually temporary. There may be swelling and discoloration, but these disappear in a few days. Scars from the incisions are initially pinkish in color but this will gradually fade with time. Patients who wish to undergo breast augmentation should schedule their surgery around a month before summer starts in order to allow time for recovery and resumption of normal activity.

Guidelines for Cosmetic Surgery Tourism

A growing number of medical tourists are availing of cosmetic surgery when they go on vacation to the Philippines. Many of them are "balikbayan" Filipino immigrants or overseas workers visiting relatives and friends. They often regard cosmetic surgery as a last-minute item on their shopping list, something which can be done during the last few days of their vacation. If a patient is seriously considering cosmetic surgery, the vacation should be planned such that enough time is allotted for the following: pre-operative consultation (1 day), pre-operative laboratory work-up (1 day), actual surgery date (1 day) and time for post-operative recovery (1 to 2 weeks). The American Society of Plastic Surgeons (ASPS) has issued an advisory that before flying, a patient should wait for five to seven days after body procedures such as liposuction and breast augmentation, and seven to 10 days after cosmetic procedures of the face including facelifts, eyelid surgery, nose jobs, and laser treatments.

If a patient has limited vacation time, having the pre-operative laboratory work up (blood tests, electrocardiogram, and chest x-ray) done prior to their vacation will facilitate scheduling of the surgery. A patient with medical insurance coverage may be entitled to a laboratory work up as part of their annual physical examination package.

Patients with risk factors such as diabetes or cardiovascular disease will also need to consult their primary care doctor, HMO physician, cardiologist, or internist, and must secure a written clearance that they are physically fit to undergo the contemplated cosmetic surgery procedure.

Once all these necessary pre-operative precautions are undertaken, the next step is to choose a qualified plastic surgeon. Patients will be taking unnecessary risks if they choose unqualified doctors to do their cosmetic surgery. Patients should also make sure that their operation will be performed in a facility that is well-equipped.
In recent years, it has been noted by the international media that the high cost of cosmetic surgery in the U.S.A., Europe and Australia has resulted in the price-driven phenomenon known as cosmetic surgery tourism. Since medical insurance does not cover the cost of cosmetic surgery and patients have to pay out-of-pocket, price has become a major factor in promoting cosmetic surgery tourism. Thus patients from First World countries have chosen the alternative of flying to countries where cosmetic surgery is more affordable.

The growth of cosmetic surgery tourism is a sign of international recognition of the capabilities of plastic surgeons who have chosen to live and work in "Second or Third World" countries.

A cursory Internet search will show entire cosmetic surgery "vacation packages" that cost less than single cosmetic surgery procedures done in the U.S.A. Many companies are offering cosmetic surgery vacation packages which generally include hospital, hotel or resort accommodations together with the cosmetic surgery. However, it must be pointed out that vacation-related activities should be avoided after surgery. Patients should avoid sun exposure, walking tours, bus tours, alcohol-intake, smoking or exposure to second-hand smoke, exercise, and water activities (swimming, snorkeling, scuba diving, water skiing, jet-skiing, parasailing, etc.). To help ensure optimal results and to limit risks and complications, patients should not treat cosmetic surgery lightly and must observe all the necessary precautions during both pre-operative and post-operative periods.

I recently attended the Annual Convention of the American Society of Plastic Surgeons held in Chicago, Illinois from Sept. 24 to 28, 2005. This annual meeting presented the latest information regarding new developments in cosmetic surgery, such as thread lifts, injectable fillers, and breast implant technologies.

There is an increasing demand for non-surgical alternatives for facial rejuvenation. Plastic surgeons can now offer patients more options for looking younger. The thread lift is gaining popularity because of the very short downtime associated with this procedure. It takes only a couple of days for the swelling to subside after a thread lift, unlike a classical facelift where a patient needs from 2 to 3 weeks to recover from the surgery.

Patients with deep wrinkles and deep grooves at the sides of the nose and mouth can now avail of a new volume filler named Sculptra. Unlike other fillers such as Restylane, Perlane, Hylaform and Aquamid which are indicated for fine wrinkles, Sculptra is used mainly for filling up deeper wrinkles and grooves.

The Anesthesiologist's Role in Cosmetic Surgery

An important factor in any successful surgery is the anesthesiologist. A good plastic surgeon will always work with only qualified MDs as anesthesiologists. This does increase the cost for the procedures but safety is always a priority, as well as the comfort of the patient during the procedure.

The experienced anesthesiologist uses modern anesthetic techniques and medications specially customized and developed for cosmetic surgery patients and procedures. The anesthesiologist chooses and gives the medications which keep you from feeling pain or sensation. These medications allow you to go to sleep gently and quickly and wake up feeling the same, usually free of nausea.

The anesthesiologist is present all throughout the surgery, monitoring and controlling the patient's breathing and blood pressure. He also makes medical judgments that protect you during and after surgery.

As I mentioned before, even patients on a budget should never scrimp on a qualified plastic surgeon and an equally qualified and experienced anesthesiologist for procedures that require more than local anesthesia.

Are you a Good Candidate for Liposuction?

To be a good candidate for liposuction, you should be in good physical health, near your ideal body weight (only 10 or 20 pounds overweight) and have good skin elasticity. You should exercise regularly and eat healthy well-balanced meals. You may have an area on your body that exercise and dieting has been unable to reduce. Such a localized fatty deposit is perfect for liposuction. Liposuction is not a cure for obesity. There is a limit of fat tissue that can be removed safely from the human body.

You should also be psychologically stable, and realistic in your expectations. You may think having liposuction will make you look like your ideal or give you a perfect body. You may think that people will treat you differently or that you will be a happier person after your liposuction. These are unrealistic expectations. Good results will help you increase your self-confidence but you won't necessarily get your ideal body.

Liposuction vs. Abdominoplasty

Many men and women struggle with unwanted fat. At times, even after months of diet and exercise, it is difficult to eliminate fat specially in problem areas such as the hips, thighs, stomach, and neck. For the abdomen and waist areas, options to remove fat include liposuction and abdominoplasty (also known as the tummy tuck). Which procedure is best?

Depending on the amount of excess fat to be removed, I would consider abdominal liposuction as the preferred initial procedure. It is an excellent method of spot reduction which involves removing fat tissue by a suctioning technique (with a special machine), and contouring the body area to obtain the desired shape. Liposuction takes a few hours and the patient is able to walk around immediately after the procedure. It may be done as an outpatient procedure, minimizing costs.

Abdominal skin contracts remarkably after liposuction. If there is loose skin still remaining after 6 months, this can be easily removed as an outpatient surgery procedure under local anesthesia only.

Abdominal liposuction entails less risk than a tummy-tuck. The surgical removal of excess skin and fat in the tummy tuck operation involves a wide curvilinear incision from the left side to the right side, mobilization of the skin from the pubic region up to the ribs, and placing of drains. It is an in-patient hospital procedure which takes from 2 to 6 hours under general anesthesia. The patient stays in the hospital from 5 to 7 days.

Breast Augmentation for Asians

Breast augmentation is very popular among Asians and many of my patients are Filipinas whose partners are Caucasians. Since Asian women are generally small framed and small breasted when compared to Caucasian women, relatively smaller breast implants may be recommended. If the implants are too large, they may not look natural and in proportion to the particular patient's body. In my initial consultation with patients, I discuss the various options that are available to the patient, including the look the patient hopes to achieve. In good cosmetic surgery, the goal is to enhance, with a result that is as natural as possible and not glaringly noticeable.

When breast augmentation gained popularity in the 80's and 90's, many women wanted oversized implants, “the bigger the better”. With these oversized implants popularized by media, the public’s perception was that ideal breasts were supposed to look large, taut and high.

Today, breast augmentation is more popular than ever. But unlike in the earlier years, patients are going back to a more natural look, with the aesthetic ideal for the female breast looking more natural in shape, soft, supple, and in proportion to the rest of a woman’s figure.

Many breast augmentation patients are women in their 30’s and 40’s (especially moms). Many mothers lose volume in their breasts after pregnancy and breastfeeding. Breast augmentation restores a natural, more youthful breast contour. Even women who have not had children are choosing smaller implants for a more realistic look.

The New Cohesive Gel Implants

Cohesive gel implants are silicone gel implants. But unlike their predecessors, the filler in cohesive gel implants has a higher amount of "crosslinker," resulting in a firmer, gummy-bear consistency that does not wrinkle or deflate like saline, nor leak as easily as the more fluid filler of earlier-generation silicone implants.

Dr. Mark Solomon, a plastic surgeon and a spokesman for the American Society of Plastic Surgeons described it by saying that even if you cut through it, the gel does not leak out.

The cohesive gel implant has a more stable shape, and works well when implanted above the pectoral muscle. This eases post-operative pain. Biologists studying the animal kingdom have discovered that animals that are more symmetrical are more likely to attract a mate. In this case. it seems symmetry is related to fitness and may be a good indicator of general health and strength.

In humans, it seems that symmetry is also a major factor in the average person's perception of what beauty is. Persons with symmetric facial features are more likely to be thought of as beautiful.

In a study done in the University of Louisville, when shown pictures of different individuals, Asians, Latinos, and whites from 13 different countries all had the same general preferences when rating others as attractive. The most symmetric faces were rated as the most attractive.

Then the pictures with the best ratings are merged together, while the less attractive photos are taken out. Each trial ends when a viewer rates the composite a 10. All the perfect 10s in the study were super-symmetric.

There is evidence that our preference for beauty may be biologically ingrained. In an experiment done by Alan Slater, a developmental psychologist at the University of Exeter in south-west England, almost 100 newborn babies, an average of 2.5 days old, were shown photographs of pairs of faces which were matched in everything except attractiveness. The infants chose to stare at the face the psychologists judged as the attractive face rather than the unattractive one.

The research showed that while it has been suggested that humans determine their concept of attractiveness based on the "average" of all the faces they see, and that features that are most close to the human average in size and shape are the most attractive, it is interesting that the newborn infants, with their very limited exposure to various human features still chose to look at the more attractive of two faces.

Study Proves People Who Have Plastic Surgery Make Better First Impressions

Many studies have shown that first impressions are greatly influenced by facial appearance but this interesting report published in Aesthetic Plastic Surgery is the first of its kind to scientifically validate that patients who underwent facial cosmetic surgery created a better first impression after they had the surgery.

Randomly selected observers were asked independently to view and grade before and after photos of patients who underwent facial cosmetic surgery. They did not know the pre- or postoperative status of the photo. The questions posed to the reviewers were based on first-impression studies used in the past.

Findings indicate that after facial cosmetic surgery, patients were graded as 31% more attractive, 27% better in social skills, 22% % more successful in dating, 19% better in athletic skills, 15% better in relationship skills, and 13% more financially successful.

Dr. Steven H. Dayan, the principle investigator of the study, explained, "There appears to be innate characteristics in all of us that attract us to definable standards of beauty. I am often told by my patients that following their cosmetic surgery procedure they have achieved a new job, an improved romantic relationship or greater personal success. I had always felt that it was secondary to a greater self confidence which had projected through, but I now have to consider that maybe there is more to it."

Cleft Palate Surgery

One of the most fulfilling types of surgery for me as a plastic and reconstructive surgeon is the cleft palate surgery. In the Philippines, there are a lot of babies born with congenital deformities such as cleft palates. The benefits of a short two hour operation last a lifetime and makes a big difference in the way these babies will live their lives. Unfortunately, many of the families these babies are born to don't have adequate finances to cover expenses, even when plastic surgeons volunteer to do the operation for free. Fortunately, organizations such as the Rotary Club of Valenzuela (Rotary International District 3800) have projects to help finance these babies' operations. Their "Bagong Pangarap Project" which started this 2005 had Baby Angel Pinoy as their first cleft palate patient. Indeed, a plastic surgeon's work of healing and helping gives these babies new hope for the future.

Plastic surgery is a specialized branch of surgery that is concerned with correcting deformities that are congenital (in-born) or acquired from accidents or aging, in order to restore or enhance form and/or function.

The origins of plastic surgery are rooted in ancient history. Until the end of the nineteenth century, plastic surgery was essentially reconstructive. With the perfection of techniques, the correction of minor defects that were congenital in nature or due to aging came to be practiced, evolving into what is now popularly known as cosmetic or aesthetic plastic surgery.

Although plastic surgery can be categorized into either reconstructive or cosmetic surgery, there is no clear distinction between the two, since there is a cosmetic aspect in reconstructive surgery, and often a reconstructive aspect in cosmetic surgery. Pioneer plastic surgeon Sir Harold Gillies defined reconstructive surgery as an attempt to restore the individual to normal, while cosmetic surgery is an attempt to surpass the normal.

As I mentioned before, media has a big part to play in the increased demand for cosmetic surgery these days. But media's coverage of cosmetic surgery also has a negative effect, as this BBC news article reports.

Leading doctors from the British Association of Plastic Surgeons say the obsession with cosmetic surgery is obscuring the real work plastic surgeons do and tends to trivialize what the specialty can do. They say some people even see them in a similar vein as hairdressers. But this perception means that some unsuitable and unqualified doctors are performing cosmetic surgery, leaving the qualified plastic surgeons to repair the damage. The doctors emphasized their work of healing and helping people, treating cancer patients burn and accident victims, and patients with congenital deformities such as cleft palates.

Here's an interesting article about how times have changed from the days when having cosmetic surgery was a dark secret. Today, cosmetic surgery has become increasingly common among mainstream Americans.

Last year, the American Society for Aesthetic Plastic Surgery (ASAPS) conducted a nationwide survey of 1,000 adults which showed that 54 percent of respondents approved of cosmetic surgery and 24 percent (30 percent of women and 18 percent of men) said they would consider it for themselves. Breast Augmentation: Is the Price Right?

When canvassing fees for breast augmentation, the uninformed patient often wonders why fees vary significantly from one surgeon to another. They often assume that it is only the professional fees of the surgeons that vary. Actually, there are a number of variables to consider when evaluating breast augmentation surgery package fees.

First of all, what kind of implant will the plastic surgeon use? There are many different kinds of implants available with corresponding prices, so make sure you aren't getting the basic, most affordable kind if you are paying a relatively high price. Most popular in the Philippines are the silicone gel implants (these are not silicone gel injections which I wrote about in a previous blog). Most of the breast implants available in the Philippines come from 3 countries: China, France, and Ireland. The most affordable implants come from China. These implants are of good quality and have been chosen by many of my patients who have limited budgets.

In the middle of the price range are the cohesive-gel implants made by McGhan corporation in Ireland and another European company, the Eurosilicone company from France. Top of the line anatomical cohesive-gel implants with multi-layered silicone shells, also made by the McGhan are on the high end of breast implant prices. So for implants alone, there is already wide range of costs.

Then, find out if the package fees include, aside from the professional fee of the surgeon, the professional fee of the anesthesiologist, cost of implants, medications and operating room expenses for either outpatient or hospital based surgery. When on a budget, you can save on operating room expenses by choosing to have your surgery done in a well equipped outpatient clinic. But for your safety and to ensure the success of your surgery, make sure you don't scrimp on the essentials: a skilled and experienced surgeon with an equally experienced anesthesiologist.

One of the most popular cosmetic procedures for Asians in augmentation rhinoplasty. Many Filipinos, especially those who reside in countries like the United States, the U.K., and countries in Europe, where nose bridges are commonly very high, get a boost of self-confidence and feel more attractive when they have their noses surgically enhanced. Indeed, since the nose occupies the prominent central area of the face, it determines to a major extent whether the face is considered attractive or not, and a higher nose bridge makes a big difference in the overall balance and appearance of the face.

A big difference, yes, but unfortunately, not always a positive difference. Augmentation rhinoplasty is a deceptively simple operation. Thus, many doctors who have not had special training in plastic surgery think they can easily do it well. After all, what could be so difficult about placing an implant to increase the bridge of the nose?

As any well trained and experienced plastic surgeon can explain, rhinoplasty is one of the most delicate plastic surgery operations. Everyone has a different nose and the plastic surgeon should evaluate the structure of your nasal bones and cartilage, the shape of your face, your skin, and your age since these factors influence his decisions about what techniques to use for the surgery to get the desired result.

Although rhinoplasty usually takes only an hour or two, this operation is difficult to learn and difficult to perform well. A skillful surgeon knows where to make incisions so that they will be practically invisible after healing. With proper technique, scar tissue doesn't interfere with achieving the desired results. A skillful surgeon also knows where to make incisions to minimize the amount of bleeding during the operation. The surgeon can have better results when there is less bleeding because he can clearly see what needs to be done. An experienced surgeon also knows how to minimize intraoperative swelling. Swelling can make it hard for the surgeon to see what the nose's final shape will be and what needs to be done during surgery.

Not only does the plastic surgeon need to be an expert on the "science" of this operation, he must also be an artist. There is so much artistry involved in the planning and execution of this operation. The best plastic surgeons carve an implant especially for each patient in the right size and shape to create a new nose that will look natural. Your surgeon must have a fine aesthetic sense such that the surgical results of your nose operation will appear natural and unnoticeable. The new nose should look like it "fits" together with the rest of your features.

With this need for surgical skills, aesthetic judgement, sense of beauty, and an ability to visualize the end result, untrained doctors find out that augmentation rhinoplasty isn't so easy after all.

Cheap Plastic Surgery, Anyone?

Type "cheap plastic surgery" in Google Suggest's search box and you'll see that so many people are searching for plastic surgery that is CHEAP. Price is definitely a factor when considering elective surgery and deciding to have cosmetic surgery is not the same as having "no choice" surgeries like appendectomies.

But any surgery is still surgery and has some uncertainty and risk involved, no matter what a "cheap" plastic surgeon says. When price is the primary factor for choosing a plastic surgeon, risks increase. Like most things in life, quality doesn't come cheaply. The success of your surgery is largely dependent on your choice of surgeon and professional qualifications and experience should be your main consideration in selecting your plastic surgeon. Even when you go to popular clinics where a number of surgeons practice as a group, take a look at the qualifications of the particular surgeon who will be handling your case.

Good plastic surgeons may charge relatively affordable fees, especially if they practice in Asian countries like the Philippines, where cost of living expenses are low and if they can keep overhead costs low by having modest outpatient surgery clinics. But think twice if the surgeon you're considering charges much lower than other plastic surgeons in the same country. 1. In order to maximize their vacation time, patients should have their blood tests done in their respective countries prior to their departure and must communicate the results to their prospective surgeon.

Silicone oil injections for non-surgical breast augmentation and noselift : a practice that should be stopped!

I have seen a number of patients who have been victimized by glib-talking people extolling the virtues of silicone oil injections. The primary attraction is the cost: "only" P40,000 for breast enlargement, which is less than half of the average cost for breast augmentation with medical-grade silicone gel implants. For those desiring a noselift, the cost is from P7,000 to P14,000. Victims are often "fast-talked" into undergoing the procedure; they are not given the time to consult with their doctor.

Noselifts and breast augmentations are two of the most commonly requested cosmetic surgeries, but there are many who find the expense quite prohibitive. Thus the lure of low-cost injections is hard to resist. When the complications arise, these victims end up spending more.

Those whose noses are injected with silicone oil end up with noses looking like the noses of lions - as the silicone oil spreads through the tissues with time, the bridge of the nose flattens out and becomes wider. The skin is often reddish, a result of chronic inflammation of the tissues.

The people who perform silicone oil injections are hard to catch. They do not have any permanent offices. They go from one beauty parlor to another; they transfer from house to house, going wherever there is someone gullible enough to fall for their sales pitch.

Preventing Complications in Aesthetic Surgery

One of the most interesting comments I heard at the 9th International Congress of the Oriental Society of Aesthetic Plastic Surgery in Bangkok came from Dr. Thomas Biggs, Editor-in-Chief of the Aesthetic Plastic Surgery Journal. He said that many of the surgical complications occur during the first week following an international congress in plastic surgery. There was an uproar of laughter from the audience following that comment, because there was indeed a grain of truth in that comment by Dr. Biggs.

Many of the presentations in any international conference deal with new techniques in surgery. Sometimes there is too much emphasis on the techniques, and not much attention is given to the indications for the technique and proper patient selection. Sometimes an overeager presentor may skip or overlook certain details. Sometimes generalizations are made, which may not be necessarily true.

The unwary surgeon in the audience who will then apply the new technique may then be faced with complications he/she is not prepared to deal with. There is a learning curve with any new surgical technique. It is encumbent on the surgeon to practice good surgical judgement. Oftentimes, one of the most reassuring experiences in any meeting is the confirmation of the validity and effectivity of old, tried and tested techniques.

New Technique for Breast Lift

In one of my earlier blogs, I wrote about the Aptos facelift innovation by the Russian plastic surgeon, Dr. Marlen Sulamanidze. At the 9th International Congress of the Oriental Society of Aesthetic Plastic Surgery, Dr. Sulamanidze demonstrated the breast lift using the Aptos technique.

During the live surgery demonstration at the Siriraj Hospital in Bangkok, Dr. Sulamanidze showed us how sagging breasts can be lifted using the Aptos suture suspension technique. Four Aptos 2-0 polypropylene sutures are placed 2 cm deep in the breast tissue and are threaded around the clavicle bone in order to mechanically lift sagging breasts.

The advantage of this technique is that it avoids the vertical or T-shaped scars below the nipple-areola complex in conventional breast lift or mastopexy operations. This is a major benefit for patients who are afraid of getting scars around their breasts.

Cosmetic Surgery Conference in Thailand

I recently attended the 9th International Congress of the Oriental Society of Aesthetic Plastic Surgery held from December 6 to 10, in Bangkok, Thailand. This congress was attended mainly by plastic surgeons from the different countries in Asia: Philippines, South Korea, China, Taiwan, Indonesia, Singapore and of course, Thailand. There were formal presentations or lectures during the first 3 days at the Shangri-la Hotel ballrooms, followed by live surgery demonstrations at the Siriraj Hospital the next two days.

The two main guest speakers from the U.S.A. were Dr. Thomas Biggs and Dr. Mutaz Habal, who both gave excellent plenary lectures.

The trend that I noted throughout the conference was the emphasis on surgical procedures that could get the maximum cosmetic results with the minimum of risks. This I will expound on later, after I finish catching up with the pile of work that has accumulated in my absence.

When to undergo cosmetic surgery?

When is the right time to undergo cosmetic surgery?

Many magazine articles nowadays encourage young patients to undergo cosmetic surgery at an earlier age. It used to be the case that patients started having facial rejuvenation surgery at the age of 4o years. However I now see patients who have such surgery in their mid-twenties and thirties. These articles claim that cosmetic surgery done at a younger age will have longer lasting results, and that major surgery at a later age will be prevented if little "nips and tucks" are done while the patient is younger.

There are many factors to consider in the timing of cosmetic surgery. Physical indications are the most obvious. However the presence of wrinkled sagging skin by itself is not sufficient for one to have cosmetic surgery. Emotional factors are important to consider. If the person is not bothered by the wrinkles and the eyebags, I see no reason to encourage them to have cosmetic surgery. I respect the decision of people to grow old gracefully. I do not have any business telling them that they should have rejuvenation surgery.

The support of one's spouse or family is also an important consideration. The results of cosmetic surgery may be "perfect," but if the spouse or family does not support the patient in his/her decision to undergo the surgery, the patient will not feel fully satisfied.

It is thus important to have a lengthy pre-operative consultation in order to determine whether or not a patient will benefit from cosmetic surgery. If a patient's expectations are unrealistic, I discourage the patient from proceeding with cosmetic surgery. For example, wives,who undergo cosmetic surgery so that their philandering husbands will leave their mistresses, will feel unsatisfied even if the surgical results are "perfect" if their husbands continue on with their philandering.

Breast Augmentation Gift Vouchers

Breast augmentation is one of the cosmetic surgery procedures that patients continue to request during the holiday months. After the operation, there is minimal downtime because patients can immediately move around in public since there is no visible bruising or swelling. The areas which may have postoperative bruising or swelling are easily hidden by the patient’s clothing, unlike in facial cosmetic surgeries like rhinoplasty (noselift), eyelid surgery and facelifts where the black and blue discoloration is there for everyone to see.

The demand for breast augmentation has even prompted some private clinics to start offering cosmetic surgery gift vouchers. The Reuters news agency released an article on November 4, 2004 which was entitled “Bigger breasts under the X-mas tree?”. This article features the sale of cosmetic surgery gift vouchers by Transform, one of Britain’s biggest commercial cosmetic surgery groups. The value of the vouchers range from 50 to 1,000 pounds and are mostly used for non-surgical procedures such as Botox and skin peels.

According to a spokeswoman for Transform, hundreds of vouchers have been sold and “most patients had already expressed an interest in plastic surgery before receiving a voucher… and were not offended by the gift”.

However, the concept of gift vouchers for cosmetic surgery has its detractors. There is concern that cosmetic surgery is being trivialized as an ordinary commodity like shoes and clothing, and that this may lead to complacency in patient care.

Some words of caution are in order for those who are thinking about giving cosmetic surgery gift vouchers. One of the most important prerequisites that a plastic surgeon must do is to determine whether a patient's expectations are realistic. Even the best results from cosmetic surgery will not guarantee satisfaction if expectations are not met, such as expecting that a philandering husband will leave his mistress and move back home.

There is also the possibility that gift vouchers for cosmetic surgery may be given to those who really do not feel the need for any kind of facial or body make-over. This kind of "gift" may even be insulting to those who don't want cosmetic surgery. Having cosmetic surgery is a personal choice and must not be dictated by others such as one's spouse or parents.
